SUMMER DATES:
June 12 - August 8 - Times to be announced on or before June 6.
1) Summer League Form completed/$50 due to the bookstore (PICK UP FORM AT BGHS)
2) Physical Form - stamped and signed by doctor, you and your guardians (PRINT)
3) Athleticclearance.com account created and forms filled out
To participate in any sport, an athlete must register and complete the athleticclearance.com forms online, have a completed current physical, have health insurance (insurance is also available for purchase). All forms must be turned in to the head coach or front office BEFORE the athlete can participate. Information and forms can be found on-line at: bolsagrande.org.
