New Student Registration Needs

Required Items for Bolsa Registration:
  • 1.  Please verify that you live in the Bolsa attendance area.  Go to the ggusd.us website, under Schools, Find My School.  Put in your address and it will tell you which school you belong to.  If you are not in our attendance area. you can always apply for a Transfer.  Please see "Transfers" for more information.
  • 2.   Proof of address:  Current gas, water, or electric bill in mom or dad’s name.  If address is not in mom or dad’s name, please have the person whose name appears on the bill write a letter stating each person from your family that resides at the address. Have the person sign and date the letter, and include it with the bill.
  • 3.  Parent must be present with photo identification (driver’s license, passport, or California ID card). If not living with parent, will need to get a Caregiver Affidavit filled out by parent and whoever student will be living with..
  • 4.  Birth Certificate or VISA of student.
  • 5.  Immunization records from a California Doctor. Use the immunization checklist or see Health Aide or Nurse. Also parents fill out a Student Health Inventory.  See below.
  • 6.  Transcripts from previous school(s)… Need all High School grades. If they have missed a semester or more, may need to see an AP.
  • 7.  If student is Special Ed or has a current IEP or 504 plan, please include a copy of the report. Also need a Special Ed Transfer into District.  
  • 8.  Withdrawal Form with Final grades from the current semester from previous school.
  • 9.  Any tests scores. Fill out Home Language Survey. See below.
  • 10.  Need Court or Restraining Orders (if applicable).
  • 11.  Students should choose elective and foreign language classes they wish to take.  
  • 12.  All 11th and 12th Graders must fill out an Age Grade Placement form.  See below.
  • 13.  If new to the district or returning after 1 year, must complete: Aeries Air.  Go to – enroll.ggusd.us.
All items must be taken care of before being enrolled.  See attached forms.

Elementos requeridos para el registro en Bolsa:

  • Por favor, verifique que vive en el área de asistencia en Bolsa.  Vaya al sitio web de ggusd.us, en “Schools”,”Find My School”.  Pon tu dirección y te dirá a qué escuela perteneces.  Si Bolsa no se encuentra en nuestra área de asistencia. siempre puedes solicitar una Transferencia.  Consulte "Transferencias" para obtener más información.
  • Comprobante de domicilio: Factura actual de gas, agua o electricidad a nombre de mamá o papá.  Si la dirección no está a nombre de mamá o papá, pídale a la persona cuyo nombre aparece en la factura que escriba una carta indicando a cada persona de su familia que reside en la dirección. Pídale a la persona que firme y feche la carta, e inclúyala con la factura.
  • 3. Los padres deben estar presentes con una identificación con foto (licencia de conducir, pasaporte o tarjeta de identificación de California). Si no vive con los padres, deberá obtener una “Declaración Jurada del Cuidador” completada por los padres y la persona con la que vivirá el estudiante.
  • 4. Acta de Nacimiento o VISA del estudiante.
  • 5. Registros de vacunación de un médico de California. Use la lista de verificación de vacunación o consulte a un Asistente de Salud o a una enfermera. Además, los padres tienen que llenar un Inventario de Salud Estudiantil.
  • 6. Transcripciones de escuelas anteriores... Necesita todos los grados de la escuela secundaria (Preparatoria). Si han perdido un semestre o más, es posible que necesiten ver un Asistente Director.
  • 7. Si el estudiante es de educación especial o tiene un IEP o un plan 504 vigente, incluya una copia del informe. También necesita una transferencia de educación especial al distrito.
  • 8. Formulario de retiro con las calificaciones finales del semestre actual de la escuela anterior.
  • 9. Los resultados de las pruebas. Llene la Encuesta Del Idioma Del Hogar.
  • 10. Necesita órdenes judiciales o de restricción (si corresponde).
  • 11. Los estudiantes deben elegir electivos y que lengua extranjera desean tomar.
  • 12. Todos los estudiantes de 11º y 12º grado deben completar un formulario de Colocación de Grado por Edad.
  • 13. Si es nuevo en el distrito o regresa después de 1 año, debe completar: Aeries Air.  Ir a – enroll.ggusd.us.

Todos los artículos deben ser atendidos antes de ser inscritos.

Giấy tờ cần có ₫ể ghi danh vào Trường Bolsa

  • Vui lòng xác minh rằng quý vị sống trong khu vực của Bolsa. Truy cập trang web ggusd.us, trong “Schools”, bấm vào “Find My Schools”. Nhập địa chỉ của quý vị và nó sẽ cho biết thuộc trường nào. Nếu không ở trong khu vực của chúng tôi. Quý vị luôn có thể xin “Transfer” để tiếp tục học Trường Bolsa. Vui lòng xem "Transfer" để biết thêm thông tin.
  • Chứng nhận nơi ở: hóa đơn gas, hoặc hóa đơn điện, hóa đơn nước dưới tên cha/mẹ. Nếu địa chỉ không dưới tên cha me, cần người đứng tên trên hóa đơn viết một lá thư xác nhận gia đình học sinh đang sống cùng địa chỉ. Sau đó người xác nhận ký tên, điền rõ ngày tháng vào thư và nộp kèm với hóa đơn.
  • Phụ huynh cần thẻ nhận dạng có hình (bằng lái, hộ chiếu, thẻ Nhận dạng của California). Nếu không sống cùng với cha mẹ, cần có Giấy Chuyển Quyền Chăm Sóc từ văn phòng Học Khu Garden Grove.
  • Giấy khai sinh của học sinh
  • Giấy chích ngừa từ bác sĩ của bang California, và hình X-quang nếu có nhiễm TB. Sử dụng danh sách kiểm tra tiêm chủng hoặc gặp Trợ lý Y tế hoặc Y tá. Ngoài ra, phụ huynh cũng cần điền vào Bản Kiểm Kê Sức khỏe của học sinh (Health Inventory – 3 Languages.pdf).
  • Học bạ của tất cả các trường cũ. Cần tất cả điểm ở các lớp trung học . Nếu học sinh nghỉ một học kì hoặc nhiều hơn, học sinh có thể sẽ cần gặp Hiệu Phó.
  • Nếu học sinh trong chương trình đặc biệt, đang có IEP hoặc chương trình 504, nên kèm theo một bản sao của báo cáo. Còn cần thêm giấy tờ thuyên chuyển chương trình đặc biệt vào Học Khu
  • Giấy ra trường với điểm số của học kỳ đang học tại trường cũ.
  • Điểm kiểm tra của tất cả bài thi đã lấy. Điền Bản Khảo Sát Ngôn Ngữ ở Nhà (Home Language Survey – 3 languages.pdf).
  • Xin nộp Án lệnh tòa hoặc Lệnh Kiềm Chế (restraining order) – nếu có
  • Học sinh nên ghi danh cho lớp tự chọn và lớp ngoại ngữ con em muốn lấy.
  • Tất cả học sinh lớp 11 và lớp 12 cần phải điền vào đơn xếp lớp theo độ tuổi (Age- Grade Placement Form to be filled out for all new 11th and 12th Graders.pdf).
  • Nếu mới đến Học Khu hoặc quay lại sau 1 năm thì phải hoàn thành Aeries Air bằng cách đi đến – enroll.ggusd.us.

Tất cả giấy tờ cần được hoàn thành trước khi học sinh bắt đầu học
